I try hard to follow the "one small goal at a time" rule. Problems arise when the work-in-progress item us blocked, usually by dependences on 3rd parties. Therefore focusing on another item "seems" to be the obvious thing to do. It is still not clear to me that this is always wrong.
Limiting Wip IMHO is like starting a diet. Requires much discipline and a focus on the whole workflow.
In Kanban (at team level) the natural way to limit WIP is to prioritize the same amount of work than you archieved the last week (like in Scrum, or XP) . I have some successful experiences coaching personal kanban and appying the same principle, but the above issue still remains
