The International Organization for Migration (IOM) is calling for action to stop the use of so-called ghost ships to transport migrants across the Mediterranean Sea to Europe. IOM says it fears many migrants will lose their lives if this risky practice is allowed to continue.

Over the past week, more than 1,000 migrants, most fleeing conflict in Syria, had to be rescued in the Mediterranean Sea. Survivors of the two so-called ghost ships in which they were traveling say the crews aboard abandoned their ships, using lifeboats, after locking in the automatic pilot as the boats neared the Italian coastline.

The aircraft was carrying 40 passengers and 6 crew members. The crew members consisted of two pilots and four flight attendants, all of whom were Italian citizens. Detailed examination of the NAV showed that the NAV mode, during its operation in-flight, can lead to a completely false LOC (horizontal alignment) or GS (vertical alignment) indication ("ON" indication). A small red flag should have appeared on the indicator when the error occurred, but it did not. McDonnell Douglas provided the investigators a letter, which it had sent six years earlier (24 August 1984) to every operator of DC-8, DC-9, C-9, and MD-80 aircraft equipped with the faulty NAV receiver, which related to the NAV switching failure mode.[2] McDonnell Douglas had held a seminar about the faulty NAV in 1985 in Long Beach, California and Alitalia had sent three pilots there. However, the message from both the letter and the seminar was not transmitted to Alitalia operating crews. They, including the crew of Flight 404, were unaware of the possible false indication.[2]

Not only did it fail to warn the pilots about the problem, but this open circuit also prevented the autopilot and the Ground proximity warning system (GPWS) from receiving the proper course and glide path deviation signals, resulting in neither instrument warning about the imminent danger. The autopilot continued to guide the aircraft according to the previously established crew inputs and the GPWS did not sound an alarm since the crew believed that they were on the correct glide path, while in reality, they were not.[2]

In the previous year, there were several accidents that were caused due to altimeter misreading. NASA research showed that, from a survey of 169 US pilots, 137 pilots said that they had already misread an altimeter at least once, 134 pilots had observed another pilot misreading an altimeter, 85% of the pilots explained that they had noticed that misreading has occurred more than once in the cockpit during their work, and most of the misreading occurred during the approach phase.[2]

Reviewing the chain of events, the investigation board made several recommendations in order to avoid similar scenarios. Improved rules for communication among pilot and co-pilot during landing have been implemented, and possibilities of misreading flight altimeters have been pointed out to pilots. Most importantly, new rules prohibit the aborting of a go-around once it has been initiated, regardless of the rank and function of the crew member initiating the go-around. Proper review of decisions and clear communications are part of crew resource management.[5]

The first reports of ghost rockets were made on February 26, 1946, by Finnish observers.[3] About 2,000 sightings were logged between May and December 1946, with peaks on 9 and 11 August 1946. Two hundred sightings were verified with radar returns, and authorities recovered physical fragments which were attributed to ghost rockets.

Investigations concluded that many ghost rocket sightings were probably caused by meteors. For example, the peaks of the sightings, on 9 and 11 August 1946, also fall within the peak of the annual Perseid meteor shower. However, most ghost rocket sightings did not occur during meteor shower activity, and furthermore displayed characteristics inconsistent with meteors, such as reported maneuverability.

Debate continues as to the origins of the unidentified ghost rockets. In 1946, however, it was thought likely that they originated from the former German rocket facility at Peenemünde, and were long-range tests by the Soviets of captured German V-1 or V-2 missiles, or perhaps another early form of cruise missile because of the ways they were sometimes seen to maneuver. This prompted the Swedish Army to issue a directive stating that newspapers were not to report the exact location of ghost rocket sightings, or any information regarding the direction or speed of the object. This information, they reasoned, was vital for evaluation purposes to the nation or nations assumed to be performing the tests.

The crowd surges toward the shore. Porco and Curtis have exited their planes and their confrontation has turned into a fistfight in knee-deep shore water. As the fistfight continues, the two pilots begin to look pretty awful, with faces that are beaten to a pulp. It's a boxing match for endurance now, with a clang on a frying pan to mark the end of rounds. With eyes almost swollen shut, both fighters' punches miss entirely. But as Gina's plane pulls up, they simultaneously knock each other out and both sink into the water. The referee starts to count. Gina calls to Porco who rises from the waves on the count of ten. He won! Gina says to the crowd, "The festival is over now. Italian forces are coming to arrest you all," and everyone disperses. "Boss" gives Fio the prize money.

In the early 1930s, Italian seaplane designers set world speed records (such as the Macchi M.C.72 designed by the Italian airplane designer Mario Castoldi). One of the test pilots killed during the attempt to set the speed record was named Bellini, the name given to Porco's pilot friend in the film.

Again, the attempts to establish a radio contact were unsuccessful and no one could be seen in the cockpit or in the cabin. Once it reached Cologne, the Cessna continued on a straight route on its last set course. Another pair of Eurofighters was scrambled from Laage, continuing to escort the ghost plane until a Danish F-16 arrived on scene to take over while the ghost jet was over the Baltic Sea.

While the aircraft crossed Swedish airspace, the Swedish authorities, planning for the worst, launched the Search and Rescue helicopters to fly in the same direction of the Citation. Shortly after, the aircraft was left without fuel and the engines failed one after another. The resulting asymmetric thrust after the loss of the first engine caused a slight turn, which combined with the ongoing loss of altitude, ultimately led to the RDAF pilots observing the aircraft entering a left-hand spiral off the Latvian coast.
