boundcond.f90: 'cT': fix usage for lnrho

'cT' calls different subroutines depending on whether it is used for
density or entropy.

bc_lnrho_temp_z sets both density and entropy ghosts, and so it is wrong
to call bc_ss_temp_z in that case.

I have also updated the documentation for the 'cT' BC.
