From the London Gazette No 62307 published on 6 June 2018:

CROWN OFFICE
Her Majesty has been graciously pleased by Warrant under Her Royal Signet and Sign Manual bearing date the 11th May 2018 to ordain and declare that Matthew Joseph Mosley, Francis Shaun Mosley, Aidan Clifford Mosley, Thomas Kieran Mosley and Monica Colette Mosley shall henceforth have, hold and enjoy the same title, rank, place, pre-eminence and precedence as the children of a Baron as would have been due to them had their Father Shaun Nicholas Mosley (commonly called the Honourable Shaun Nicholas Mosley) survived his Father Nicholas Baron Ravensdale and thereby succeeded to the title and dignity of Baron Ravensdale. And to command that the said Royal Concession and Declaration be recorded in Her Majesty’s College of Arms.

I don't understand why the Queen doesn't just issue a Royal Warrant making this automatic in all cases. Has there ever been an instance where it has been requested but refused?

I imagine where a request has been refused it hasn't been made public. It may be that The QUEEN is not in favour of 'rubber stamping' this kind of thing and prefers instead to consider each request on its merits.

Does the new Peer make the request, or is it their siblings collectively?

I think it's the person seeking the courtesy style who petitions for it.

I'm scraping the old memory bank here but I think there's standard form of words (a prayer) which is submitted via a lawyer.

In this age, does it really matter if you had the style of a baron's son or daughter in any way?? Are there any real events at which the order of precedence for the peers is followed ??

I imagine because its a 'special warrant' in each instance. No one has any rights to something they never enjoyed. The precedence derives from being the child of a peer. Not that you could have been the child of a peer in some senario. The same has happened very very very exceptionally for the wives.
