Job Summary

 

The Peer Specialist provides non-clinical support for enrollees and provides the perspective of lived experience for the interdisciplinary Care Coordination Team. In working directly with enrollees, the Peer Specialist helps engage enrollees, and acts as a role model with all enrollees by sharing their experience and competency in recovery, resiliency, healthy behaviors and healthy lifestyles.


Essential Functions

  • Serve as a resource on recovery principles and experience as a member of interdisciplinary Care Coordination Team. Provides input on enrollee’s successes and barriers in engaging in self-management of conditions and self-directed care.
  • Engage with enrollees on an individual basis as identified in the Care Coordination Team. Identifies recovery goals, teaches and mentors enrollees in problem-solving skills. Teaches enrollees how to identify and overcome fears, learn coping strategies, and encourages self-management and relapse prevention. Encourages socialization with family and friends as well as participation in community based groups such as churches, training, and volunteer work.
  • Provide outreach and engagement support for hard-to-reach enrollees. As assigned, facilitate enrollees’ making and keeping of appointments, build confidence in using transportation options, and assist enrollees on specific goals such as follow up after a transition in care, adherence to medications, or socialization. Accompany enrollees to appointments, as requested.
  • Organize and facilitates group activities for enrollees, as assigned. Leads WRAP classes and other group meetings. Shares leadership of programs on chronic condition self-management and healthy lifestyle topics. Serves as a role model in living a healthy lifestyle.
  • As requested, engages with community and consumer organizations as well as providers regarding the role of peer specialists and recovery-oriented programs in Magellan Complete Care. Supports MCC’s connection with community organizations, the justice system, ALFs and others by being friendly, responsive, and responsible.

Non-Essential Functions

 

Candidate must be able to work effectively and prioritize in a fast-paced environment in order to meet the demands of the organization. Candidate must role- model competency in recovery and ongoing coping skills.

Qualifications

 

Education: Associates Degree
Experience: 1-3 years
Industry: Any community based program involving social services or health care
Job Specific: Florida Board Certified Peer Recovery Specialist 

An equivalent combination of experience and education is acceptable.

Other 

* Knowledge of recovery principles and tools, such as wellness recovery action plans 
* Ability to communicate effectively, both written and verbal 
* Ability to represent strengths and needs of consumers to clinicians 
* Ability to summarize and document in care coordination plans 
* Able to prepare and make presentations and facilitate discussions
Licenses, Certifications, etc.: Current certification from the Florida Certification Board. Requires automobile in good working order.

Preferred Qualifications
Education: Bachelor’s degree preferred.
Experience: 2 years of post-certification experience as a Peer Support Specialist
Whole Health training, preferred
