A 50-year-old woman with a 15-year history of type 2 diabetes and hypertension, a 25-pack-year history of cigarette smoking before quitting 3 years ago, and a family history of early myocardial infarction presents for evaluation. She has a blood pressure of 144/84 mm Hg (similar to home blood-pressure readings during the past month) while taking lisinopril 20 mg daily, an LDL-cholesterol level of 88 mg/dL and a triglyceride level of 180 mg/dL while taking atorvastatin 40 mg daily, and an estimated 4.3% risk of developing atherosclerotic cardiovascular disease in the next 10 years. Which one of the following strategies is most appropriate to further reduce this patient’s risk for cardiovascular disease?
