What steps will reproduce the problem?
1. Just try to render the attached pdf.

What is the expected output? What do you see instead?
Some fields completely missing and QR code not rendered

What version of the product are you using? On what operating system?
Latest version of pdfium from git-reposity. Windows 10

Please provide any additional information below.

Are you building with XFA support enabled after downloading from git? Otherwise, this is may be related to pdfium:1090

Additionally, is this something that broke recently (i.e. was this successful with older versions of PDFium)?

Yes, I am using pdfium with XFA and V8 support.

I do not know, if it worked before. But I have tested with different tools incl. Foxit and the only tool able to handle this file was Adobe.

As tsepez@ mentioned https://bugs.chromium.org/p/pdfium/issues/detail?id=1615#c1, this issue might have the same cause as issue #1615. If one of the issues is fixed, we should verify the other one.
