Hi all

FYI - some info about the CDM guidelines for CH update.  Jill Kelly is looking for suggestions of innovative practice; “useful tools and case studies that demonstrate good models of care, the innovative use of technology and forward thinking of modalities in chronic disease management” (DHHS, PCP update Oct 2015).   Jill indicated that the examples don’t need to be full case studies at this point, just some points.

 

(BTW - Maybe we can collate this type of info via our Networks on Base-camp into the future?).

Dear Catherine and Bethea

thank you for your interest in reviewing the updated guidelines which we hope to have available in early 2016.  The Primary Health Unit is keen to "stretch the sector" and make the guidelines functional for the next five years.  Therefore if you have any examples of innovative practice I would be interested in hearing from you.  I don't need a full case study or report just yet, unless you have one, as I'm just collecting suggestions for inclusion in the guidelines.  I've attached a document that outlines the expectations and timelines.
