At our last PCP Statewide meeting we agreed to circulate a survey to members regarding the amalgamation of the Service Coordination & Integrated Chronic Disease networks with regards to:

 

·         nominations to be on the Early Intervention & Integrated Care Network Secretariat

·         2016 PCP Forum
