I’m writing to seek your expert advice please.
I’m new to PCPs and joined the Central Highlands PCP in August this year in the service coordination role.
I’m engaging with service providers in the region and working to encourage improving service coordination practices. It’s becoming clear that organisations in our region have varying levels of understanding around what good service coordination practices look like.
I’ve put together a presentation that I can go through with organisations but it’s targeted at the lowest level of understanding. I’ve discovered that it is fairly challenging to get across on what service coordination is all about in 5 mins.
Then it dawned on me…….there are many experienced and very knowledgeable people across the PCP network and someone may have already come across or developed some great resources to explain service coordination to the masses (in an engaging way). Of course, presentations will need to be adapted depending on the audience.
I’m thinking I would like to put together some ‘real life’ scenarios to demonstrate what great service coordination practices look like and I have found some interesting resources to assist.
If anyone is happy to share their presentations or any other useful resources., I’d appreciate any advice you can give.
