Adoption of IHE PCD in Brazil and the 2073 Law of 31/08/11 of the health ministry.

Dear IHE PCD Technical Committee,

the 2073 law in Brazil specifies the SNOMED-CT standard to support semantic interoperability between information systems. How can we adopt the IHE PCD in Brazil if this domain specifies HL.7 V2.6 using the semantics of the IEEE 11073? Is it possible adopt the IHE PCD with HL.7 v.2.4 and SNOMED-CT standard?

It is possible to create mappings of US DOC NIST supported RTMMS (Rosetta Terminology Mapping Maintenance System) IEEE 11073 observations to SNOMED-CT.  A number of IEEE 11073 observations may be more detailed for which there are might not be directly equivalent SNOMED-CT terms.  As such there may be a loss in detail when mapping from 11073 to SNOMED-CT so both observations might be included in the values within the message so you have the mandated SNOMED-CT value and there is no loss in detail as the 11073 value is available for reference.

You are reading it correctly.

 

There is a part of the IHE Technical Frameworks that permits national extensions or differences.  For example, this permits HL7 v2.4 to be used in Japan as mandated by the government when the PCD TF expects HL7 v2.6 (with according loss of OBX-20 Observation Site which originated in HL7 v2.6).

 

A National Extension for Brazil could be proposed that prioritized SNOMED-CT over 11073 as mandated by the government where there are SNOMED-CT equivalents else would use 11073 and would also permit HL7 v2.4 instead of v2.6 (with according loss of OBX-20 Observation Site which originated in HL7 v2.6).

I read IHE PCD domain Technical Framework, Version 5.0, Volume 2 and highlight the part below (page 107):

 

  "...For the IHE PCD transactions, implementations shall use the terms defined in the current version of the Harmonized Rosetta Terminology (Volume 3 of the Technical Framework  contains further details and references on the Rosetta Terminology Mapping as well as important information on system responsibilities regarding terminology). The Rosetta codes are based on terms from the ISO/IEEE 11073 Nomenclature where available, and where the Nomenclature does not currently contain a matching term, gives provisional vendor-neutral terms to be submitted to the IEEE 11073 Upper Layers Committee as  suggestions for adoption into the Nomenclature. If term cannot be found in this way and a matching term is available in LOINC, then the next preference is to use the LOINC term. If LOINC also does not support a term then SNOMED CT or another coding scheme recognized by the HL7® standard takes precedence if a matching term is available."

 

In this case, I believe there is an obligation to use the terms of nomenclature IEEE 11703 in IHE PCD and only  I can use the SNOMED-CT in case of missing term in IEEE 11703. If you all terms exist in IEEE 11703 could not use the SNOMED-CT.

 

Our library uses SNOMED-CT terms and would like to adopt medical equipment with IHE PCD's declaration of conformity using HL.7 v.2.6 without changing the nomenclature of the library.

 

I would like your opinion on my interpretation.
