Keep in mind that we can only make payment directly to the veterinarian's office. Please give us one week advance notice of the appointment. Our approval is contingent upon you completing of the following:

• Please check with the clinic to determine if it will accept a credit card payment over the phone the day of surgery.  Our Treasurer calls credit card payments in by 5pm (EST) on the day of surgery.  Please inform the clinic of this, so there's no confusion on the day of surgery.
• Please confirm the person's name on the appointment/account at the vet's office.
• If the clinic will only accept a check, please send us the full address and contact name to attention the check to. Please allow PBRC a minimum of one week prior to your appointment so the clinic will be sure to receive the check in time for surgery. 
• Please let us know if there is a contact person we should speak to on the day of surgery.
• Please notify us immediately if any information changes. Failure to notify us could result in a disqualification of assistance.

This offer of assistance expires on 1-29-10.

Yes, that is exactly why you did not get a confirmation email from either of us. The only thing I have on the calendar is for Maka's appointment on Friday, 11/5, which I will be calling in.
