Medications given by enema primarily treat vata dosha, which is the main etiological factor in the manifestation of diseases. Vata is the force behind the elimination and retention of feces, urine, bile, and other excreta. Vata is mainly located in the large intestine. Purisha dhara kala, the colon membrane, is related to asthi dhara kala, the membrane of the bone tissue. Asthi (the bones) are important sites of vata dosha. Hence, medications given rectally affect all the tissues up to bone tissue.

Basti is often administered in two stages: an oil stage and a decoction stage. In the first stage, five ounces of warm sesame oil is introduced into the rectum and retained for ten minutes. Then, without expelling the oil, an emulsion of oil and an herbal tea is introduced and retained for thirty minutes. This mixture consists of five ounces of sesame oil together with sixteen ounces of a tea made from herbs steeped in hot water and then strained and cooled to body temperature or slightly warmer.

The enema bag may be suspended three feet above your position. After filling the bag with the appropriate fluid, allow all the air to be expelled from the tube. Lie on the left side, left leg extended and right knee flexed. Lubricate the tip of the syringe with oil or ghee. Clean and lubricate the anal orifice with oil and check that there are no hemorrhoids, fissures, or fistulas. Carefully and slowly insert the tip into the rectum and release the clip allowing all the fluid to enter, then remove the tip. Assume a hands and knees position and elevate the buttocks, thus relaxing the colon. Firstly tap the colon area from the sigmoid, through the transverse colon, to the ascending colon and caecum. Then periodically massage the colon in this counterclockwise motion.

When fluid has been retained for the appropriate length of time, sit on the toilet to allow the passage of the fluid and fecal matter. You may wish to wear a sanitary pad for a few hours following basti in case there is some seepage of residual oil. It can be good to eat an hour or so after a basti, but only when hungry.

In this article, we will help you understand the importance of Basti treatment in Ayurveda. Basti in Ayurveda is said to be among the primary therapies; we can also call it the king of all Ayurveda treatments. Often in our clinical practice, we are amazed to see the results of Basti.

Basti is one of the most powerful of the main five procedures of Panchakarma. Charaka, as well as many other Ayurvedic scholars, have unanimously praised the features of Basti. Basti is said to be equal to half of all Ayurvedic treatments.

Basti is the introduction of herbal decoctions and medicated oils into the colon through the rectum. While it directly affects the colon, it is not a localized or symptomatic treatment. It is a highly specialized procedure to achieve a therapeutic goal and must be carried out in hospital settings by an expert.

Basti treatment has been found to have scientific benefits for the nervous system. B-group vitamins, which are crucial for nervous function, are produced by the beneficial bacteria in the large intestine. In degenerative nerve diseases, the protective myelin sheath is lost. The nerves can be nourished by administering fatty substances and healing herbs through enema therapy, and the production of vitamins in the large intestine can be increased. This aids in the regeneration of nerve fibers and promotes overall nerve health.

Usually, in our practice, we have observed Vata can also cause an imbalance of chakras resulting in various problems. Basti helps to bring the chakras back to normal, thus balancing the energy levels. It removes unwanted materials from the cells and improves metabolism at the cellular level. So one can achieve health at all three levels (physical, mental, and spiritual) by Basti alone.

I have taken basti for paraplegia ( accident in Dec 2022) in Dec 2023 and in January 2024 I got tingling sensation in both the legs , so I continued upto 21st Feb .But no improvement thereafter but the tingling sensation is continuous in my legs should I go for another session of basti treatment ?

It seems to be high Vata as a result of the surgery. Once this Vata is brought down by right therapies including Basti , Nasya, Shirodahara , Shirobasti etc., you should feel better. Also a Vata pacifying diet and lifestyle will be important.

We cannot promise a cure for this condition. Sometimes multiple sessions need to be done to achieve results. This requires
treatment sessions every year for 2-3 years/ twice a year for 2 years.

HimVeda symbolizes efficacy and reliability of Ayurvedic treatments for disorders which are posing a challenge to the modern system of medicine. Ayurveda is as simple as mother nature itself. Simple yet not simplistic We have kept our working principles simple to match this simplicity.

This scheme provides the funds for annuity and funds for laying down the electricity and other facilities in the colony developed under the MGGBY Scheme. Fund of Rs 30 Cr. was approved during the year 2022-23. An amount of Rs. 7.44 crore (released on 20.06.2022) + 4.62 crore (released on 29.08.2022) were released as annuity funds in lieu of land utilization of the Gram Panchayats during the 2022-23. A Special Task Force has been constituted under the chairmanship of the Block Development & Panchayats Officer concerned to analysis the Bastis developed under this scheme. This committee will also conduct a survey of these bastis and will submit its report of the essential development needed.

The decoction enema is a combination of many ingredients as mentioned above. These ingredients should be mixed in a specific sequence to formulate homogenous mixture and achieve maximum therapeutic efficacy. The sequence of mixing of decoction enema is as follows. Honey and rock salt should be mixed first. Then unctuous substance such as oil or ghee is added and mixed. Medicinal paste is added next to it and finally the decoction is added.[Cha.Sa.Siddhi Sthana 3/23] This process acquires the state of emulsion. Honey is a natural emulsifying agent and is preferred for the preparation of basti because of its stability and therapeutic properties.[7]

On the day of administration, patient is advised to pass natural urges and should not be hungry. [Cha.Sa.Siddhi Sthana 3/17] Local massage with oil and sudation is done over abdomen, low back, gluteal region and thighs of the patient. The enema mixture is prepared and filled into the basti apparatus (basti yantra). In current clinical practices, two types of basti apparatus are used to administer decoction enema.

Patient is advised to lie in left lateral position with left leg extended and flex the right leg at knee and hip. The anal orifice and the basti nozzle or catheter are lubricated with oil. Then the basti nozzle or catheter is inserted slowly and gently through the anal canal.

The basti bag is squeezed in uniform pressure to pass the basti medicine in rectum. It is not emptied totally in order to avoid passage of air through basti apparatus. Then the nozzle is removed slowly. [Cha.Sa.Siddhi Sthana 3/18-20]

After all enema medicine is in the rectum, the patient is advised to lie in supine position till he feels the urge for defecation. When the patient feels the urge, it shall pass. The time duration the administration of medicine and its evacuation is called as retention enema. The decoction enema is expected to come out within 48 minutes after administration. In a study, the retention time of enema with alkali (kshara basti) in maximum number of participants (60%) is reported to be 5 minutes.[8]

Sequential elimination of stool, pitta, kapha and vata, increased appetite, lightness in body, increase of strength and attainment of normalcy, passage of frothy and whitish mucous in stool are considered as features of optimal administration of niruha basti. [Cha.Sa.Siddhi Sthana 1/41] [Ka.Sa.Khila Sthana 8/75-77][9]

Headache, pain in cardiac region, anus, urinary bladder and genital organs; edema, coryza, griping pain and nausea; retention of flatus and urine and dyspnea are features of inadequate administration of niruha basti. [Cha.Sa.Siddhi Sthana 1/42-43]

Aggravation of vata dosha due to diminution of kapha and pitta dosha, numbness, bodyache, mental fatigue, tremor, insomnia, debility, black outs, insanity and hiccups are features of excess administration of niruha basti. [Cha.Sa.Siddhi Sthana 1/19-20]

Unctuous enema is contraindicated in diseases where purification is indicated like skin diseases (kushtha), obstinate urinary diseases including diabetes (prameha), obesity (sthaulya)[Cha.Sa.Siddhi Sthana 1/36]; the diseases with ama (undigested state) like acute fever, anemia, rhinitis etc. It shall not be given to patients on empty stomach and those who have low digestive capacity. [Cha.Sa.Siddhi Sthana 2/18]

Unctuous enema is administered immediately after meals. [Su.Sa.Chikitsa Sthana 37/54][3] On the day of enema, the patient should take little quantity of food, part less than his regular diet quantity. The patient is advised to walk for few steps and to pass the natural urges if any. [Su.Sa.Chikitsa Sthana 37/57-58][3] The lukewarm medicated oil or ghee is filled in the enema syringe attached with catheter. In some conditions, fine powders of Anethum sowa (shatapushpa) and rock salt are added to oil. [Su.Sa.Chikitsa Sthana 37/63][3]

Unobstructed expulsion of fecal matter mixed with oil, feeling of lightness in the abdomen, clarity in sense organs, improved quality of body tissues like blood etc., sleep and proper manifestation of natural urges are observed after optimal administration of basti. [Cha.Sa.Siddhi Sthana 1/44] The features are similar to that of oral intake of unctuous substances (snehapana). [A.H. Sutra Sthana 19/53][6] Proper movement/functioning of vata dosha and improved digestion are also observed in optimal anuvasana basti.
