Is there a way to clear the input fields in a record without having to save/refresh and then select don't save change?

Not 100% sure of the goal here, but possibly the Add Individual Record action (one of the actions available for a Record object) is what you want? It removes the existing record before creating a new one.

If that is not what you are after maybe try posting an annotated screen shot describing how it works now and what you are trying to achieve.

When I change the Refresh/Clear button action to add individual record, refresh, etc. it still opens a popup window asking if I want to save changes to XXX.

If a user enters data into a number of fields on a form and then for whatever reason wants to start over with a clear form, is there a single stroke method to clear a form?

It will always prompt them if they have unsaved changes. They can simply click Don't Save on the dialog panel.
