Paragon Patient Kiosk / Medical History

Wondering if anyone has a 3^rd party integrated with Paragon that will allow us to begin to capture information provided by the patient. We are looking for something that will allow the patient to enter information into a Kiosk or another type of template application / device for things like family history, social history, family members, reason for visit, etc.. and have the information flow back into Paragon.

Is there such a thing?
