Upload redirect works intermittenly, 7 out of 10 times only

we are hitting an extremely serious issue with upload, what happens is
that on some situations and it can not be isolated exactly when,
upload never redirects user, and ajax check keeps returning the same
status of starting,

It'd be great if we can isolate the conditions which cause this. One thing we have seen in the past is if you try and upload and it fails, then retry with another video, the id used for the upload progress is the same id. What we've done in the past is generate a new uuid with each upload, e.g. http://gist.github.com/312451
