Do you have any insight on anaphylaxis to the pneumovax, and what testing I should pursue for further risk of this patient to anaphylaxis with other agents? The patient had immediate anaphylaxis to the vaccination within one minute of administration in my office. I was able to witness the entire event. Facial angioedema, facial flushing, coughing, wheezing, hypoxemia. She had obvious systemic reaction, no doubt. I administered IM epi IM Benadryl and Albuterol and symptoms reversed (angioedema remained for hours). She had received pneumovax 7 years prior by another provider. She had dwindling titers and recurrent infections, the rest of the immune eval was normal, so that is why I pursued pneumovax in the first place. She has no other history of idiopathic anaphylaxis, reaction to vaccines, mast cell activation, chronic hives, or food allergies. The review of the package insert states that the vaccine contains 0.25% phenol. No other active ingredients are listed except for the purified capsular polysaccharides for strep pneumo. Could this be phenol allergy? How would I confirm phenol allergy? What should I warn the patient about in regards to future phenol exposure?

I just had my second pneumovax shot about 5 hours ago. The first time, about 5-7 years ago, I had awful upper arm pain for about 3 days. I've had no other problem with any other vaccines including my yearly flu shot. Well, I'm sitting here typing this in incredible pain! This is worse than the first time. I can move my arm, but it's extremely painful to do so. I'm wondering if the nurse did it too...
