Need urgent tech help - creating a form to add folks directly to a google group

hey guys,

One of the primary objectives of Darshakji's meetings in larger groups is to increase our membership.

People would want to sign up for the chapter's local google group.

Our experience in bay area has been that its most efficient to have people  signup from their smart phones right in the hall before they leave - while they are still very inspired.

Can somebody help with creating a form from which they can be directly added to the googlegroup?
