Information for International Students: Submitting Letters of Recommendation

International students need to follow the steps listed below when
requesting and submitting letters of recommendation:

1) download the reference form (available at http://admissions.osu.edu/apps/pdfs/refer.pdf)
2) give one form to each of your referees
3) ask each referee to complete the form
4) instruct each referee to attach the form to their letter of
recommendation
5) instruct each referee to put the form and letter into an envelope
6) instruct each referee to seal the envelope and write their
signature across the seal

At this point, the referees can either mail their letters directly to
me or they can give the signed and sealed letters to you to mail to
me.

If there are extenuating circumstances, I might accept a letter of
recommendation via e-mail as long as it is mailed directly from the
referee who composed the letter and the e-mail is sent from an
institutional e-mail address (like osu.edu and not like @gmail.com).

No applicant should complete the reference forms themselves (I'm
speaking specifically about the sections where the referees rate the
applicant's performance).
