What is the default widget in the top left corner of the navigation screen (attached 121m). It appears to be a navigation arrow with a value, but how does it differ from the arrow and value (231m) at the very top of the screen?

Found the answer myself.  It's the 2nd left turn.

The big one is the first instruction (go right/left, whatever)
The smaller one is the second  instruction following after the first instruction, so the next tunr left/right what ever.
So in your image: Go right after 231 meters, and then go right after (another) 121 meters.
