Hi PCPH colleagues and friends,

 

We are recruiting for a PCP at one of our Employer-Based Primary Care Clinics – Cisco Life Connections Health Center, which is located in San Jose on the main Cisco campus.

 

It is a unique clinic because of its integrated care model, which includes primary care, physical medicine (PT, chiropractor, acupuncture), behavioral health, optometry, and pharmacy.  It serves Cisco employees and dependents, and is a valued benefit because of the coordination and optimization of care we can provide for patients and is in many ways a model for care. 

 

Having worked there for several years, I developed lasting relationships with patients addressing both their chronic disease management as well as urgent care needs.  Appointments are 30 or 60 minutes and there is inbox support over the weekends.

 

We are hiring for a 1.0 FTE but are open to less if you are interested.
