Dear Friends

Kindly give your valuble suggestions as to what can be done now

52 yr. old male Road accident Oct 2021

Had a R femur shaft communited jn of middle and upper third

During nailing additional communition extended proximally to upper thrid’

Now 9 months down he still cannot bear weight fully

Xray July 2022 shows varus defmormity, most likely non union upper third

What is to be done now- I was thinking of long DCS screw plate with with head screw inserted 20 degree caudad direction after nail removal and fixing to distal shaft first and then proximal and bone grafting. Inserting another nail and correcting the varus may not work

Nail removal and new closed nailing with correction of angulation - more medial and posterior entry point.
