?CONGENITAL APLASIA PATELLA

Dear friends

Kindly suggest possible diagnosis and if needed treatment

9 yr old boy- since 3 yrs mother noticed difficulty in climbing stairs and getting up from floor

Runs and plays around and since the pandemic he has put on weight and now his heel is slightly above the floor,

He was seen in a couple of hospitals

Slightly overweight boy, normal gait and active knee extension, mild equinus no dorsiflexion.

Clinically ? extensor mechanism subluxing on flexion on R side and flet laterally L side

X-rays- absent patellae, No features of nail patella syndrome.  AP standing shows mild lateral shift of both tibiae at the knee, Hypoplastic lateral femoral condyles

Is it Congenital aplasia of patella- Is an MRI needed to know the status of extensor mechanism?

Any treatment needed?
