Dear Friends

Kindly suggest what can be done

Case of R foot subtalar fracture dislocation, crush injury foot with injury to tibialis posterior , FHL, FDL tendons posterior tibial artery, crushing of posterior tibia nerve due to road accident in Feb 2021. Treated in a major hospital with wound debridement, fracture dislocation reduction. K wire fixation and external fixator

Pt has come now with deformity and absent sensation in instep area of the foot-rest of sole sensation normal especially weight bearing area and has clawing of toes

Pt has now pain on the medial side on full weight bearing walking and is also concerned about deformity.

He has mild hindfoot valgus, forefoot abduction and as expected loss of arch with flattened medial border of foot.

Xrays show mild hindfoot valgus, forefoot abduction in AP view and in LAT view talo navicular subluxation.

CT scan shows a mild calcaneal valgus and foot abduction

I would like to know whether I should do a calcaneal osteotomy with opening wedge cuboid osteotomy or think of a triple fusion

 Attaching  x-rays and clinical pics

 Regards
