This 16 yr boy has history of infection in his left knee which was treated with antibiotics with keeping limb straight for 6 months elsewhere.

Now he has totally stiff knee without infection.

His demand is movement of knee………!

can we help him?

I am sending his xray pics & MRI clips.

Kindly give your valuable opinion on it.

After explaining to the patient and his parents, can you attempt to remove the bridge.The quadriceps is contracted for the only last 6 months and it may yield with active physiotherapy. He is already 16 and hence growth disturbance may not be a problem.
The consent should be proper.
It will be a heroic surgery but one can be meticulous and careful during the surgery

Dear Dr.Iyer,
even if we remove the cartilage bridge what will prevent it from reforming ?
for an instance if we keep it distracted with ilizarov apparatus what will be working in place of  cruciate ligaments as stabiliser ?
if anyhow we get informed consent,........ is there any previous case to prove ourself ethical ?

Dr Mahajan,
It will (may) not reform because there is no infection now. There is no previous case to my knowledge.
We can put up the case in other sites (othopod, show my case) to get more opinions
