Dear Friends

Kindly advice what is to be done and my experience with Polio is rather limited nowadays as the number of cases we seen are very few

33 yr old case of post polio residual paralysis.

[L] ankle and foot equniocavovarus deformity. 3 cm shortening

Fixed deformity

Muscle power- quadriceps Grade V, Ankle dorsiflexors- Grade 0[ could not see acting tibialis anterior], Plantar flexors Grade V

EHL and EDL grade IV and Tib post and peroneals Grade III

What is to be done- he would like to walk better- [ pt has come now this late due to financial reasons].

My plan is either do a soft tissue release and  followed by distraction with ring fixator and triple fusion  OR a Lambrinudi procedure along with IP joint big toe fusion and Jones transfer of EHL

Attaching clin pics and x-rays

Kindly advice

Yours sincerely
