Here is my lesson plan with attached Google Form. 

It is a short research project of various sight reading examples online where students find examples online and then analyze them.  I would have liked the students o be able to upload a screenshot into google forms but don't think google form supports uploading photos. I did find that someone was able to do it but it involved writing an actual script for google form and that way above my level at the moment.

Great work. I think you take a relatively simple lesson plan, for a tough concept (IMHO) to teach. Your use of Google Forms is flawless. I've been trying to explain for years the possibilities for gForms...and to me the best uses are these simple evaluations in which we have students document learning and self-assess.
