One possibly fun idea, sort of related to what we're already doing with the crosswalk inventory:

I'm envisioning 'gamifying' it as well, where you'd get 'points' based on feedback (which you could give via the app after walking somewhere). You'd tell the app, "I walked!" [get points], then it'd ask how your walk went [get points for every category of feedback, e.g. a point for answering 'did you have room?' and one for 'Was crossing easy?']. Finally, it'd give you double points for taking some action to improve the situation [Send a note to the engineering dept. or call the city council office.]
On top of that, you could take photos of the problem areas, and submit those as well, and we could create a web app with a map of all the checklist responses and photos. Between that and getting people to notify the government about a problem, this would hopefully be a really useful advocacy app.
Curious what people think of the idea, and if anyone knows of any revenue stream that could pay for the development of it (assuming it isn't a terrible idea).
