Looking for outcome surveys

Hi,

I'm looking for a pre and post session outcome survey that I can use
at my internship site. We need to find a way to quantify the
therapeutic work we're doing with teens for the school district.
Something that would measure how a person is doing in the beginning of
therapy and then at the end of therapy.

Anyone have something they use or have any ideas?

Would this be for a client centered outcome ?  If so go to talkingcure.com Scott Miller and Barry Duncan's work on therapeutic change that focuses on the client's perception of the outcome not the clincian's.

Regarding outcome surveys, here is a voluntary UCONN Graduate Survey
on clinician use of research and outcomes data in their practice:
