Electronic Health Care records for use in a primary health care setting

I´m planning to try out one of the available opensource alternatives
and saw this list http://nursingassistantguides.com/2009/50-successful-open-source-projects-that-are-changing-medicine/
here.

Several of those mentioned here seem promising -but I´d like more info
preferably from someone with practical experience.

What I am looking for is a system with integrated
healthrecords,laboratory results, prescription and timemanagement/
appointment modules.

Any help would be greatly appreciated!
