Content-Rewrite default expectations change

I'd like to propose that we change the default expectation of the content-rewrite 'include-url' param so that instead of everything(*) it assumes nothing.  In essence, forcing gadget developers enumerate the files they wish to optimize.

This should have no functional ramifications, as gadget writers should be assuming that every container has this feature.

The reasons for this are due to high barriers that implementations face in supporting this feature well for everything by default (an everything that is increasingly hosted properly on a CDN).  Rather, it would be better for the gadget author to specify which resources should be rewritten as they move to a phase of development where optimization is important.
