Dear Group members

I am a radiologist from a government university hospital in Türkiye. I have an OpenREM system and am trying to fix optimization issues from digital radiography equipment. We use OpenREM and ClearCanvas PACS software in the hospital. 
My problem is, exposure parameters of the radiographies are visible in PACS software, but I can only see the DAP meter result in OpenREM results. The x ray machine is a Shimadzu Digital radiography. 
How is it possible to have exposure parameters in OpenREM. Is there any intervention we can do with this issue? Many thanks.

I have a few questions that might help us to understand the issue:
  1. Can you tell us which model of Shimadzu X-ray you have? 
  2. Can you tell us how the DICOM information gets to OpenREM? Do you send directly, or does OpenREM query the PACS?
  3. Do you know if OpenREM is reading DICOM images or if it is reading an RDSR (DICOM radiation dose structured report)?
My initial thought is that maybe the system is creating a very basic RDSR that is being read by OpenREM, but in ClearCanvas you are looking at the image headers.

Dear Kaan

Thank you for your reply which you sent just to me - I am replying to the group so that other people can hopefully help and learn too.

You have told me that you have the following X-ray systems where you can see kV, mAs, EI, TI and DI in PACS (ClearCanvas), but not in OpenREM:
  • Shimadzu Radspeed Pro (fixed room)
  • Mobileart Eco MUX 10 (mobile)
I think you have set up OpenREM to query-retrieve the data from PACS, and that you use a Linux server.

You are also having trouble with DLP and CTDIvol for two of your CT scanners - maybe we can deal with that afterwards! You are successfully collecting data from other CT, mammography and cath labs.

Questions:
  1. Which version of OpenREM have you installed?
  2. You say you are not a code person - do you have any physicist support or someone who can download the study direct from the X-ray system or from Clear Canvas and work out which type of objects you have - do you just have the images (DX or CR) or do you also have RDSRs from these systems? If you have both, the next task would be to look in detail at the RDSR.
  3. If you can perform a test study or QA study on one of these systems (no patient information) - you could send the study to me via email (not to the group) and I could have a look.

Dear Ed,
I need to ask my IT friends about the version of Open REM in the morning, but we installed it less than one year ago; I will clarify it.

We have an IT team and PACS company supporting the ClearCanvas, which is very skilful. Until today it is not obligatory to have a medical physicist in radiology in my country, but MPEs from RT and NT are not working with these optimization projects, so they do not help me too much. But the IT and PACS teams may intervene if some configurations will be necessary on the OpenREM software. 
I will send this message to my IT and PACS friends to pull them into the discussions. 
I sent the last message just to you by mistake; it is better to share the experience, I agree.
I can and will send you a sample of an anonymized patient image and a .dcm file of an image from PACS. It is a Shimadzu image. Many thanks again.

Thank you for confirming the version (1.0.0b2) and sending through an anonymised image.

I have had a quick look in a DICOM viewer and importing it into OpenREM, and I see the same situation you have described - all the information is available in the headers, yet OpenREM is not importing it. 

I will have a look through and get back to you.
