Needless to say, without further info, video or at least images and log, we cannot help.
Also please tell us what your ambient light situation is.
SOT23 are know to be problematic, as they can deteriorate to
match the minimum area bounding box at an odd angle:

And it is easy to imagine that even a slight change of ambient light, perhaps lighting the bevels of the pins from the side, can make it go over the tipping point.
If nothing else helps, the Vision Compositing a.k.a. Multi Shot Vision can help by aligning pins separately on each side of the body. Of course this comes at some cost of extra alignment time. It is described and demoed here:
