Initial encounter form issues - ART info

ART information is entered many disconnected places on the form and
some of these are duplicates.
It would be better to organize this into one coherent section.

The following separate sections should be consolidated.
1. Transfer in on ART
2. (no title... bottom of first page) ART status and eligible, ready,
etc.
3. ARV start-stop
(this section has an additional problem in that you can't click both
'Eligible and ready to start ART' in section 2. and 'Start ART today'
in this section.)
4. ARV drugs (this section is partially duplicative and has
ambiguously phrased questions.


The form logic needs to be tested and improved. For instance, I was
able to submit the form even though I had checked eligible and ready
to start tx but had not entered a regimen.
