So far I have not seen the behavior you mentioned, so without additional
information
it will be hard to reproduce the error and fix it.
You would help us if you could answer the following questions:
1) Could you see the 3 entries before? For how long (i.e. have you used OI
Safe for a
couple of days or weeks, or just minutes?)
2) Did you do anything special before it stopped working? (like install a
new
version, or type the wrong password repeatedly or..?)
3) Has the problem ever since occured? Or did it only happen once?

I think I was able to reproduce though it might be the expected behavior as
well
1. Open OI Safe
2. Enter the Master Password and click on Continue
3. A list of categories is displayed (by default Business and Personal are
displayed)
4. Select Personal
5. Click on the Add (Password Key and +) icon.
6. A description detail form appears
7. Click "Back" button once or twice till you get to the screen where all
the records inside the "Personal" category are displayed
8. An empty record appears in the list view and the Toast "Entry Saved"
also appears.
Awaiting clarification whether this is a bug or not and whether this is
what the original poster meant by empty entries
