OpenHDS update

Hello everyone,

The OpenHDS has been updated with the following changes:

* Adding dynamic fields to existing entities (Physical Data
Extensions).

* Adding dynamic fields to existing entities (Entity-Attribute-Value
Extensions).

* Declarative Configuration for enforcing custom codes, validation
logic, id generation, and other misc options.

* RESTful web services are now implemented.

* Special Study application has been integrated with OpenHDS.
