Modifying the User Interface

Hello everyone, I know there was discussion at the work shop about
minor user interface issues. Specifically:

I have attempted to solve these issues. You can review the changes at
our demo site: http://openhds.rcg.usm.maine.edu/openhds

The Baseline form, as well as the Welcome page, Location, Individual
and Social Group amendment forms have been adjusted to use the
modified layout.
