-LUT COE file

I cannot locate the subject file.  The IP Description states that it is included in the package.  Can you point me to the COE file for the LUT that contains the IP packet header file?  I need to set up:

Destination MAC address, Source MAC address, Destination IP Address, Source port, Destination port, Header Checksum.  I'm not using the JAVA application for setup.
