How are you connecting to the mount in PHD2? The iOptron Commander uses
the ASCOM interface. You should select the iOptron ASCOM mount in PHD2.
And, do not close the iOptron Commander window when it opens. It is
OK to minimize the iOptron Commander, just to not close it or click the
'X' in the corner.

I think there's a separate checkbox in TheSkyX driver config window that says to enable pulse-guiding. I think it's disabled by default, so you'll need to find it and check it. I'm not sure of the exact terminology they use, but I'm pretty sure it's there in the UI.
