Find a list of all US OCD ids

Where can I find the list of all US OCD IDs along with their associated entities in a CSV or some other format? 

I am trying to build a questionnaire that enables a civic representative to:

1.  select the state, and then 
2. the type of entity they represent, and then
   
3. Enable them to select the specific District they represent

So that they can enter some specific information about that district, that we can then link to the associated OCD ID, e.g.

1. User select from the drop-down list of states
2. User select Ohio
3. User then presented with List of different entity types, state, state legislative, Senate, congressional, city, school district, county.
4. User selects City
5. System displays List of cities in Ohio.
6. User selects Columbus and then is given a prompt enter information about said district.
7. The information is now associated with unique OCD ID for Columbus, so that I can look up at a later date using the OCD ID 

Hopefully this makes sense, and someone can provide some insights as to where one would acquire this data.

Thanks, but I haven’t been able to find a file that has all of the US division IDs. I find links to corrections, but not a CSV or something with all of the Division Ids.
