Hello. There are missing icons on the .exe and other files. This problem both on the file icon pack and on the win default. In some cases win default pack has some icon on .exe (see screenshot)

Icons for exe are provided directly by windows, regardless of icon pack, so something else might be wrong. Let me know if this happens even after restarting.
If OC asks Windows shell for the icon of a file and Windows never responds with the icon, the thread for extracting icons will be locked and no other icons will be returned. I have not seen that happening so my guess would be a faulty file or link. If that keeps happening and you don't see where it stopped extracting icons I could make a custom build that will log all filenames of extracted icons into a text file, and then you will be able to see which file causes that issue, and I may be able to find a workaround.

I don't know what happened with my message, so here it is again in case you didn't get the one before this:
This version logs everything related to icons into a file in \Log folder. Extract it in a separate folder, run, make sure that the version in corner is  3.1.18.1 and browse folder just until you see icons stop showing. At that point, right-click on tray icon and click e Open Log and scroll to the end

You will see something like
[12:33:55 INF] C:\Users\milos\Downloads\OfficeSetup.exe Extracting True
[12:33:55 INF] C:\Users\milos\Downloads\OfficeSetup.exe Done True
[12:33:55 INF] C:\Users\milos\Downloads\OfficeSetup.exe Extracting True
[12:33:55 INF] C:\Users\milos\Downloads\OfficeSetup.exe Done True

I am looking for one with "Extracting" without  following with a line "Done" or "None", or an error message.

This version is very slow, so icons will slowly appear, so even if you stop seeing icons, wait a few seconds to confirm that it is not writing to a file that is causing this.

You can close OC, delete the log, and run it again to check if the next time another file breaks it. Don't use if afterwards, as the log file will grow huge

So at this point you have tried opening additional folders and there were no additional icons shown? This all looks correct. Which program do you have for handling archives? Reinstalling that program may be able to help, as the last thing I see is that .7z icon extracted.
You can try opening c:\windows\ in both browser panes, and restarting program, then navigate to other folders that don't have .7z or other archives. When it breaks you can check again log if the last file that has icon extracted has the .7z or other archive extension

From the log it doesn't seem that it is even trying to load other icons. You may try clearing windows icon cache
Other than that, some 3rd party security software might be blocking OC's access to other files
