reorg melanocytic nodes

I have three recommendations regarding the organization melanocytic lesions:
1. Put Melanoma of Unknown Primary (MUP) under Other -> CUP
2. Rename Melanoma (MEL) to Skin Melanocytic Lesion, especially because it currently has Congenital Nevus (SKCN) as a child node, which is not malignant!
3. Put Atypical Nevus (AN) under Skin Melanocytic Lesion
