Data Output Error?

I just sent a test survey to 4 people. The system said all
respondents answered the survey, however, our output data only
contains responses for one person. Please help, and let me know if
there is an email that I can send the output file to, since I cannot
attach here.

Also, is there a way to allow multiple responses to a single question?
