Added DICOM structured reports (SRs) and voice transcripts commands

Hello all:

Based on input/feedback from @theEHRGuy (via Twitter) I added:

- DICOM structured reports (SRs) - dsr
- voice transcripts - vts

These are just proposals at this stage. As discussed we can keep
everything as a proposal until someone actually implements it.

If anyone has any feed back, heartburn etc, let the group know here.

Kind of interesting... this far this entire format has been created as
a result of collaboration over Twitter, Skype, and HealthCamp!

Please let me know if we need to add any commands. Also I think the
1st page is getting huge and I'm not sure if ou guys can edit it. We
should break it into different pages so everyone can edit the
individual wiki pages.

Also I changed this group settings a bit so we can eliminate spam.
