Command Reference

I finally began the rather large task of breaking the main page into
separate wiki pages. (Now everyone can add/edit to the wiki.) This
will eventually replace the big ugly tables on the project's front
page.

I only developed the TOC & stubbed out the bloodpressure docs.

Anyone want to help convert over material to the new command reference
wiki docs? (It takes a while bc of the need for wiki markup.)
