Abstract: There is a widespread push by government and private payers to make the prices of health care services more transparent to consumers. The main goal is to promote more effective consumer shopping; secondary goals include promoting provider competition and reducing pricing variation. There are several headwinds opposing these efforts. One problem is that there may be several valid reasons for why price variations persist. Another is that provider (and other health care) markets are not very competitive, and sometimes widespread information about prices may make them even less so. A third is that price discrimination may be economically efficient. Any analysis of price transparency must take the specific market setting into account. This chapter analyzes markets characterized by monopolistic, oligopolistic, and competitive conditions to determine when and under what economic and managerial circumstances price transparency will be useful.
The purpose of this course is to apply economics to an analysis of the health care industry, with special emphasis on the unique characteristics of the US healthcare markets, from pre-hospital to post-acute care. Mark Shepard is an associate professor at Harvard Kennedy School of Government and a faculty research fellow at the National Bureau of Economic Research (NBER). His research studies health care markets, with topics at the intersection of health, industrial organization, and public economics. Much of his work focuses on competition and policy design in health insurance markets, particularly in public programs like the Massachusetts/ACA health insurance exchanges and Medicaid managed care. Mark received his PhD in economics from Harvard University (2015) and his A.B. in applied math from Harvard (2008). He was a Post-doctoral Fellow (in Aging and Health Economics) at the NBER during the 2015-16 academic year before starting as an assistant professor in 2016-17. Before graduate school, Mark spent a year working at the Brookings Institution's Engelberg Center for Health Care Reform (2008-09).

We conclude by offering two policy recommendations based on our analysis. In the near term, the state should prioritize expanding access to treatment in underserved areas. This would require working with physicians and hospitals in underserved areas to encourage providers to obtain the waiver required to prescribe opioid treatments to their patients. We note that Vermont offers an excellent model for expanding access to opioid treatment. In the long term, the state should focus on improving the labor market outcomes of residents in areas severely impacted by the crisis. Specifically, we recommend that the state focus on improving educational investments in as a way of deterring drug abuse and overdose, particularly noting the substantial evidence linking early childhood interventions on improved employment outcomes later in life.

Dr. Shen is an assistant professor (tenure-track) at UNC School of Medicine, with appointments in the Department of Psychiatry, UNC Neuroscience Center, and Carolina Institute for Developmental Disabilities (CIDD). Dr. Shen is a translational neuroscientist who directs a research program with three aims: (1) To conduct longitudinal neuroimaging and deep phenotyping of infants to identify early markers and therapeutic targets for neurodevelopmental disorders (NDDs) including: autism, Fragile X syndrome, and Angelman syndrome. (2) To reverse-translate these findings in children to mechanistic studies in animal models and in vitro cell models. (3) To forward-translate this research to clinical trials delivering novel therapeutics to individuals with NDDs.
