I updated my Bluetooth driver via Windows update, and in the middle of the update, my mouse stopped working. "Devices" in settings said Bluetooth was turned off, but there was no way to turn it on. I checked Device Manager, and there is no Bluetooth device available. However, there is an "Unknown USB Device (Device Descriptor Request Failed)".
I tried uninstalling the drivers and updating to 22.200.0 from the Intel website, but to no avail. I was also unable to rollback the driver version. Windows Bluetooth troubleshooter also fails to find a Bluetooth device.
Power cycling didn't have any effect. I switched to a wired mouse and reconnected my Bluetooth mouse through a Logitech unifying receiver, so I am able to interact with the computer, but I'm still unable to use any other Bluetooth peripherals.
I also tried restarting the Bluetooth Support Service under services.msc. No effect. Also uninstalling the "Unknown USB Device (Device Descriptor Request Failed" from Device Manager just brings the device back later, and I'm unable to manually install a driver for it. Platform is Windows 10 64 bit.
Done all the troubleshooting I can find on-line with no luck. I think I'm going to physically remove the AX200 card, reboot then uninstall everything I can find relating to it, and then re-install as it was working perfectly for the past several years. What a pain!
OK kudos for this needs to go to ... this is total voodoo but I had exactly the same problem with Asus ROG Strix X570-E Gaming motherboard ... added and removed various versions of driver, disabled and reenabled bluetooth in bios etc. etc. etc - tearing my hair out until this weird fix really did fix it ... so my final steps were:
Well I'm bowled over! This worked for me! BT is back and working fine. I guess the soft re-start they do is not enough and making sure all the power has dissipated from the MB is what seems to be needed.
I have a question!
I been hearing about this power supply solution but it seems to work for PC's or laptops where you can remove the battery.
I have an Asus X571GT where you can't simply remove the battery.
Will holding down this power button solution work as well? It doesn't factory reset anything right?

There is also a setting in Windows update that will allow it to search for updates to 3rd party and hardware. This is one way to get the correct driver automatically. I would prefer to manually find it though.

I have installed the Windows 10 Technical Preview on my mid-2011 MacBook Pro 13-inch. I had been running Windows 7 with no problems. The installation had been done using Apple's BootCamp software. The problem is that, in Windows 10 Device Manager, my Bluetooth USB Host Controller (found in "Other Devices") shows that no drivers are installed. Worse yet, when I click on Properties for the device, I get:
Since W10 is not officially supported, the closest appropriate driver is in Boot Camp Support Software 5.1.5621 package. Can you download and check the Bootcamp.xml and check the version of Broadcom drivers in the package? Do not install it.
BTW with whatever driver I can connect my iMac to my MBP in Windows 10 Bootcamp with Bluetooth without problems, with full pairing and codes exchanged. (BTW2 Keyboard and mouse connect fine to any Mac, just not in Windows 10).
Part of the issue here is that different Mac's require different solutions, I think. The OP was looking for a solution for a mid-2011 MBP whereas most of the successful fixes here are for later machines.
My problem - I've running Windows 10 on my Late 2011 MBP in Bootcamp. I'd upgraded from Windows 7 where I'd been running bluetooth sucessfully. However, the Late 2011 MBP is not supported by Apple for Windows 10 though many people are using it successfully nevertheless. After some fiddling with Windows Services I got my Magic Mouse and Apple Keyboard working okay. However in the past little while, I noticed that I could not connect to my BT speaker or other devices. In fact, the speaker wasn't discoverable, but only on this machine. In the course of taking online advice, I deleted my Magic Mouse from Bluetooth, and then it wasn't discoverable either, and my keyboard had also dropped off for no apparent reason.
So the search for drivers.... I did a uninstall and reinstall using the default Apple driver, but the problem persisted. I tried ridhyop's solution, which made the mouse and keyboard discoverable (improvement), but pairing was too flaky to work. I search on!
I tried rolling back my installed driver, and then running it, and I tried to install the drivers manually, but couldn't find them independently of the package. Also not sure why a Wireless driver will help Bluetooth.
There is collaboration between BT and Wireless. W10 is problematic on a 2011 model, if clean installed. We are trying to find a driver that will work. What is the possibility of installing W7/W8.1 on this Mac temporarily?
I'm not enthusiastic for that, given I have a 95% working version, except for Bluetooth, sound and some keyboard issues which I understand are common. I had upgraded fromWin7 to Win10. Let me try a few other options and get back.
First, do not rely on windows to update your bluetooth driver. Find out what bluetooth card or chip your computer is using and go to that manufacturers website to get their latest updated driver. Once installed it should work.

If you connected to newer first you have to reboot the buds before connecting to older. As the buds default to connecting to last connected first means you have to turn Bluetooth off of last connected.
I have repeatedly connected my Life Q20 Headphones to Windows using my bluetooth dongle. Windows even recognizes it and connects. However, no sound comes through and the Headphones never show up as an option in the Sound Settings.
I have disconnected and forgotten the headphones, and then re connected them. I have also installed this driver ( -firmware). I have also tried resetting as said on this thread (Cant pair headphones Life Q20 with Windows 10).
None of these things worked. Is there a solution? I have been using the headphones wired through an AUX cable for the past couple of months and they have worked flawlessly, but it is right now with this bluetooth issue that I am facing a problem for the first time.
Most threads on similar issues has the issue as their Bluetooth driver. Update the Bluetooth driver and it may resolve it. There was ome person that stated that had to update it with a generic Bluetooth driver.
So I tried installing a bluetooth driver through a link ( -Wireless-Bluetooth-Driver-for-Windows-10-64-bit-for-Intel-NUC) then a straight update from Bluetooth Devices in the Control Panel. Neither solution has worked.
After the headphones get connected, there is a 10 second period where they stay connected (but my sound control panel cannot find them to use) and then they automatically disconnect and refuse to connect again.
Reset the headphones and then pair with the PC, that fixes the issue most times. If not then delete all pairings and turn all Bluetooth off of all other devices it was paired with, then reset then pair just with the PC.

Well i got bluetooth dongle now and Q10 is working fine without any issues.
It was frustrating going all through those steps posted in different forums and not getting result.
thx you all of you and these forums who are there to help.
and thx again @The_Professor
The lack of a robust set of documentation on steps, decision tree, process of elimination, is the ideal answer. Perhaps in some situations a BT dongle is the answer, well that should be known chipsets to not work.
