On the front page there is a section to fill a maximum of 3 doctors.
I have 3 doctors, so i called my local office to ask about adding my
other 2 doctors to the form, by the actions of my doctor I was going
to see... as it is a form MY DOCTOR MUST FILL OUT.
My rep. told me that my doctor-I was going to see can add my other 2
doctors to the form.
After getting home, I noticed that my doctor only added themselves to the form.
I have heard rumored info that only GP (family doctors) can add other
doctors to this form and that other specialized doctors are not
legally allowed to do so.
The doctor I went to is not a GP.
ODSP told me to have my doctor fill it out. Well, my doctor like i
said only put their individual name and info on the form.
So, since the doctor signed the form, I would believe it is a fraud
or against the law to modify such a form once the doctor has signed
their John Hancock.
So, before I sent the form to ODSP, I stapled a shortly letter to the
top of the form stating that my doctor didn't add the other 2
supplied doctors to the form and that 'here is the additional info'.
Today, i received the same form back in the mail saying the form is
incomplete and the information MUST BE on the form.
What are the legal rules of doctors adding other doctors to the form
in this location, without the other doctors consent?
If only that one doctor can add their individual name, why are there
3 doctor entries and one medical signature at the end of the form.
Do I need 3 forms???

Turns out my Dr. is just being lazy., ODSP is saying....
I have 2 other doctors to see in the next month and she made note of
that for now.
My ODSP rep was very co-operative and respectful of where I stood in
the matter and couldn't believe the laziness of my doctor's
willingness to add the other 2 doctors to the form.
Get this.
My rep is sending me 2 more forms.
If one of the last 2 fill out all doctors, I only send in that one,
but if all don't add my other 2 doctors, I will send in all 3 .
She has put a 'tab' on my file in regards to this. Not for any other
issues. Just for the idio-ticy of getting my form updated in the future.
The other 2 doctors are a neurologist and respirologist for
breathing, etc. (spelling?).
ODSP isn't mad at me. She understood my situation, and basically was
irritated with the doctor.
Yes, i can understand that some doctors or lower regulated people
that can't be accounted for on a form. I would think a natural-path
isn't included on these forms either.
This is the 2nd time I have been through this duplicate form
stuff. ODSP like i said has put a note on my file now about updating
my MSN files from now on.
ODSP even said,' we want to make sure everyone is on the form so you
are getting everything you deserve.'
Go figure.... ODSP looking out for my benefits?
