Attached you will find the guide for FMLA, STD  (Short Term Disability), and Additional Information.

 

This guide is for all – Directors, Managers, Supervisors, and Employees.  This guide provides directions for Directors, Supervisors, and Managers.

This guide will provide you with an understanding of FMLA and the responsibilities needed to successfully follow through on FMLA claims.

 

If you do not refer to this guide and/or contact the SR HR Program Manager when you are not clear, you are at risk for inaccurate information.

 

For Managers/Supervisors, who have the responsibility of entering time in Kronos, please refer to the attached PDFs that will assist with how to locate the codes and enter the codes in Kronos. 
Please be sure to read the section “What Codes to Use in Kronos” in the FMLA guide, which assists with understanding this process.

  1. Do I need to speak to my Manager/Supervisor or the Sr. HR Program Manager (Tanya Parker), before I contact Unum to open up my FMLA/STD case?
    1. No, you do not.  Please follow the steps to open up your case with Unum.  This ensures that there is no delay in meeting your needs.  You can open up a case on the website 24 hours per day, and the Unum representatives are available Monday through Friday, 8am to 8pm.

  1. Does my manager have to know the medical details for my FMLA/STD claim?
    1. You do not have to share the medical details with your manager.
