I am formally requesting an independent external review of Orange County Adult Protective Services’ handling of the Lexi Beaver / Abernethy case and related supervisory and case-management practices.

This request is based on:
– documented harm confirmed by criminal conviction,
– findings in a court-appointed Guardian ad Litem report,
– investigative reporting by The News & Observer,
– prior NC DHHS findings involving Adult Services leadership (2020), and
– significant loss of public confidence among families seeking protection.

This review must be external to Orange County DSS and county legal counsel, and should assess:
– intake screening decisions,
– supervisory judgment and home-entry protocols,
– case-manager escalation standards,
– coordination with law enforcement and medical reporters, and
– ongoing fitness for authority in supervisory roles.

This request is not to relitigate facts already established, but to evaluate administrative responsibility, risk exposure, and prevention of recurrence.

Please advise who will be conducting this review, the scope, and the anticipated timeline.

I request a written response for the public record.
